Q: How do I make an appointment?
A: If you are a new patient please send me a message through email (jeff@jeffchenmd.com), or leave me a voicemail at 650-353-7022. This way all questions you may have can be answered prior to your first visit. Follow-up appointments can be made using my online booking system which I will provide the link to by email.
Q: Do you take insurance?
A: I do not participate in any insurance plans and am considered out-of-network. However, I am able to provide Superbills after each appointment with the appropriate billing/diagnosis codes. This can be submitted to your insurance company for potential reimbursement.
Q: What are your fees?
A: Please contact me through email at jeff@jeffchenmd.com, and I will send you current information regarding office policies and fees.
Q: How long is an appointment?
A: Initial evaluation sessions vary depending on the age of the patient and complexity of the situation. Please visit the Services tab for a more detailed description of the evaluation process.
Q: Are there forms I have to fill out prior to my visit?
A: Prior to your first visit, you will be asked to complete forms, including patient information, office policies and credit card authorization. The required forms will sent to you through email.
Q: What if I can't make my appointment?
A: If you cannot keep an appointment, please call or email within 72 business hours to cancel, or reschedule. A full session's fee will be applied to missed visits or late cancellations.
